All nomination forms, with supportive data should be submitted no later than Saturday, November 30th, 2019. Nominators must complete the appropriate nomination forms as accurately and informatively as possible.

NOTE: Please be sure to include a photo, and attach appropriate supplementary information such as press clippings, correspondence, coach’s reference, etc. to your submission.
